Key Responsibilities
  • Evaluate patient engagement and risk criteria to identify individuals who could benefit from case management.
  • Build trusting relationships with patients and their families, serving as an advocate and mentor to empower them to become more independent and self-sufficient.
  • Collaborate with other healthcare providers to connect patients with necessary resources and services, promoting patient independence and self-advocacy.
  • Administer screening tools to assess patient behaviors, social needs, and risk factors for health and behavioral health problems.
  • Maintain accurate and timely records, documentation, and data entry to support patient care and program evaluation.
Requirements
  • Associate's or Bachelor's degree in Social Work, Public Health, or a related field.
  • Bilingual proficiency in Spanish and English.
  • Three years of experience in social service, community action programs, or healthcare delivery and systems.
  • Excellent communication and interpersonal skills, with the ability to work independently and collaboratively as part of a team.
  • Strong organizational and time management skills, with attention to detail and ability to multitask.
  • Proficiency in MS Office and other relevant software applications.

  • Annualized salary range of $55,000 - $60,000.
