Key Responsibilities- Evaluate patient engagement and risk criteria to identify individuals who could benefit from case management.
- Build trusting relationships with patients and their families, serving as an advocate and mentor to empower them to become more independent and self-sufficient.
- Collaborate with other healthcare providers to connect patients with necessary resources and services, promoting patient independence and self-advocacy.
- Administer screening tools to assess patient behaviors, social needs, and risk factors for health and behavioral health problems.
- Maintain accurate and timely records, documentation, and data entry to support patient care and program evaluation.
- Associate's or Bachelor's degree in Social Work, Public Health, or a related field.
- Bilingual proficiency in Spanish and English.
- Three years of experience in social service, community action programs, or healthcare delivery and systems.
- Excellent communication and interpersonal skills, with the ability to work independently and collaboratively as part of a team.
- Strong organizational and time management skills, with attention to detail and ability to multitask.
- Proficiency in MS Office and other relevant software applications.
- Annualized salary range of $55,000 - $60,000.
- Great benefits package and bonus opportunities.
- Hybrid remote and in-person work arrangement, with regular on-site attendance in Warwick, RI.
