Nurse Practitioner Job Description at Charter Oak Communities

We are seeking a skilled Nurse Practitioner to join our team at Charter Oak Communities. As a licensed healthcare professional, you will be responsible for providing medical care to residents at Scofield Manor. This includes conducting assessments, planning and administering medication, and supervising care.

Job Responsibilities:
  • Conduct regular health assessments of residents and develop individualized care plans.
  • Administer medications and ensure accurate records are kept.
  • Provide wound care, nebulizer treatments, diabetic testing, and other LPN functions as needed.
  • Coordinate medical appointments, tests, and lab work for residents.
  • Schedule transportation for medical appointments and ensure timely arrival.

Requirements:
  • State of Connecticut Licensed Practical Nurse (LPN) certification required.
  • Minimum four years LPN experience, preferably two in a managerial role.
  • Experience working with diverse populations and providing a range of services.
  • CPR certification mandatory.
