Job Summary

The RN Case Manager plays a vital role in ensuring the seamless transition of patients from hospital to home or next level of care. This position requires a skilled and compassionate nurse to assess, develop, and implement discharge plans that meet the unique needs of each patient.

Key Responsibilities
  • Conduct thorough assessments of patients to identify their discharge planning needs and develop individualized plans to meet those needs.
  • Collaborate with the interdisciplinary team to ensure that patients receive the necessary care and support to achieve optimal outcomes.
  • Identify and escalate gaps or barriers that may impact patient care and develop strategies to address these issues.
  • Participate in quality improvement initiatives to enhance the discharge planning process and improve patient outcomes.
  • May also be responsible for utilization management, ED case management, and liaison work with post-acute providers.
Requirements
  • Graduate of an accredited school of nursing.
  • Three years of acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of practice.
