Job Summary

SSM Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring patients receive the highest quality care and are discharged safely and efficiently.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to meet their unique needs.
  • Collaborate with interdisciplinary teams to identify and address barriers to discharge.
  • Ensure compliance with Condition of Participation (CoP) discharge planning.
  • Identify opportunities for improvement in standard work and participate in quality improvement initiatives.
Requirements
  • Graduate of accredited school of nursing or education equivalency for licensing.
  • Three years' acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of practice.
