Job Summary

SSM Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring patients receive the best possible care and support throughout their healthcare journey.

Key Responsibilities
  • Assess patients' discharge planning needs and develop personalized plans to ensure a smooth transition to the next level of care.
  • Collaborate with interdisciplinary teams to identify and address gaps in care, reducing the risk of readmission and improving patient outcomes.
  • Utilize evidence-based practice and clinical expertise to make informed decisions and drive quality improvement initiatives.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Graduate of an accredited nursing program.
  • Three years of acute care experience.
  • Current RN licensure in the state of practice.
