Job Summary

As a RN Case Manager at SSM Health, you will play a vital role in ensuring patients receive the best possible care and support throughout their healthcare journey. This position requires a skilled and compassionate nurse who can assess, develop, and implement effective discharge plans to meet the unique needs of each patient.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure a smooth transition to the next level of care.
  • Collaborate with the interdisciplinary team to identify and address any barriers or gaps in care that may impact patient outcomes.
  • Develop and implement strategies to reduce readmissions and improve patient satisfaction.
  • Participate in quality improvement initiatives to enhance the overall quality of care and patient experience.
  • Provide education and support to patients and their families to promote self-care and independence.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in the state of practice.
  • Three years of acute care experience, preferably in case management or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to identify and address complex patient needs.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
