Physical Therapist II

We are seeking a highly skilled Physical Therapist II to join our team at Houston Methodist Sugar Land Hospital. As a Physical Therapist II, you will be responsible for evaluating and developing the therapy plan of care for a broad range of diagnoses and acuities.
Key Responsibilities- Evaluate patients to identify their level of function and determine the treatment plan.
- Develop and implement the physical therapy treatment plan, providing service to complex patient populations.
- Provide education to patients, patient's family members, and/or caregivers, based on learning needs.
- Collaborate and communicate with the healthcare team to modify the patient's treatment plan, progress, and discharge planning for achievement of identified patient specific goals.
- Assess patients regularly, monitors progress towards treatment goals, modifies goals, and plan or program as indicated.
- Graduate of education program approved by the credentialing body for the required credential(s) indicated below in the Certifications, Licenses and Registrations section.
- Two years of experience as a Physical Therapist; or completion of HM or comparable Physical Therapy Residency program.
- BLS - Basic Life Support (AHA) AND PT - Physical Therapist - State Licensure with the Texas Board of Examiners.
- Demonstrates the skills and competencies necessary to safely perform the assigned job, determined through on-going skills, competency assessments, and performance evaluations.
- Ability to effectively communicate with patients, physicians, family members and co-workers in a manner consistent with a customer service focus and application of positive language principles.
- Strong assessment, organizational and problem-solving skills as evidenced by capacity to prioritize multiple tasks and role components.
