Key Responsibilities- Screen referrals for appropriateness and perform admissions to facilitate the transition of patients and families into the hospice program.
- Coordinate the interdisciplinary care team for assigned patients and implement the plan of care.
- Provide primary nursing care for hospice patients, regardless of the setting.
- Assume full responsibility for professional management of assigned hospice patients' care related to their terminal illness.
- Teach primary caregivers skills needed to care for patients in their residences.
- Work closely with attending physicians and other team members to develop appropriate pain and symptom management.
- Complete nursing assessments and admissions of patients.
- Maintain clinical records to meet federal, state, and agency regulations and guidelines.
- Supervise hospice aides and certified nurse aides, making supervisory visits at least every 14 days and face-to-face visits at least monthly.
- Assume all on-call responsibilities as assigned.
- Participate in Performance Improvement Program.
- Participate in and present inservice programs, team conferences, and staff meetings.
- Provide skilled nursing care to assigned patients/families as outlined in the plan of care.
- Participate in the development of patient/family interdisciplinary care plans.
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
- Excellent communication and organizational skills.
- Ability to handle confidential matters.
- Ability to work a flexible schedule.
- Commitment to the Hospice concept and standards.
- Ability to interact with a variety of different age groups.
