Job Summary:
A Licensed Practical Nurse (LPN) at Autumn View Health Care Facility is responsible for promoting the health, wellness, and illness prevention of our residents through daily nursing and personal care endeavors.
Key Responsibilities:
- Ensure the care and safety of all residents in the nursing unit
- Ensure that individual care plans are followed
- Administer and document medications, treatments, and other nursing services
- Makes a count of controlled substances on the assigned nursing unit
- Records current condition of assigned resident in EMR
- Receives and gives an accurate report of the resident condition at shift change
- Communicates to the nursing team and medical provider as indicated and required
- Assists in the orientation of new staff
- Maintains competency in all the tasks needed by CMS
Requirements:
- Minimum 18 years of age
- New York State (LPN) Licensed Practical Nurse licensure
- Strong written and verbal communication skills; able to follow written and verbal instructions
- Basic computer knowledge, PCC e-MAR experience preferred
- Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
