Job Summary

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Autumn View Health Care Facility LLC. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents.

Responsibilities
  • Provide direct patient care and support to residents, including administering medications, performing clinical procedures, and documenting patient data.
  • Collaborate with healthcare team members to develop and implement individualized care plans that meet the unique needs of each resident.
  • Conduct regular assessments and evaluations to identify and address any changes in a resident's condition or needs.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care and support.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Graduate nurse currently eligible for licensing in the State of New York.
  • Minimum 18 years of age.
  • High school diploma or equivalent preferred.
  • Strong written and verbal communication skills.
  • Able to work well with and show respect towards fellow staff members, physicians, residents, and family members.
