We are seeking a skilled Graduate Practical Nurse (GPN) to join our team at Autumn View Health Care Facility LLC.
Job SummaryThe successful candidate will be responsible for providing high-quality patient care, working under the direction of a registered nurse, and contributing to a collaborative team environment.
Key Responsibilities- Provide direct patient care, including administering medications, performing clinical procedures, and documenting patient information.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Ensure patient safety and well-being by maintaining a clean and safe environment.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
- Graduate nurse currently eligible for licensing in the State of New York.
- Strong written and verbal communication skills.
- Ability to work well with others and demonstrate a caring and compassionate bedside manner.
- Physical endurance for frequent activity and ability to lift a minimum of 20 pounds.
