Job Title: Case Manager

About the Role:

The Case Manager will be responsible for providing professional nursing care for patients in a primary care setting, according to nursing practices, agency policies, and regulations. This role will involve assessing patient needs, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure high-quality patient outcomes.

Key Responsibilities:
  1. Plan, provide, supervise, and document professional nursing care for patients in accordance with the NC nurse practice act, physician orders, and established policies and procedures.
  2. Assess patient needs, develop, and implement individualized care plans, and collaborate with healthcare teams to ensure high-quality patient outcomes.
  3. Complete required OASIS Comprehensive assessments at admission, recertification, resumption of care, and discharge.
  4. Work with interdisciplinary teams and patient providers to prevent re-hospitalization of patients.
  5. Be aware of patient insurance requirements and ensure authorization for all services have been obtained.
Requirements:
  • Associate Degree or Diploma in Nursing required.
  • Registered Nurses are not required but encouraged to enroll in a DUHS-approved BSN program after completing two years of service.
  • Current RN license in NC or compact RN licensure from participating states.
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
  • 1-2 years of med-surgical, critical care, or home health/hospice/infusion experience preferred.
