Job Summary:The Ambulatory Care Nurse will work collaboratively with a multidisciplinary team to provide comprehensive care to patients with cardiovascular conditions. This includes assessing patients, developing and implementing individualized care plans, and educating patients and families on disease management and self-care.
Responsibilities:- Provide direct patient care in an ambulatory setting, including assessment, planning, implementation, and evaluation of patient care.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Provide education and support to patients and families on disease management and self-care.
- Collaborate with the healthcare team to ensure seamless transitions of care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Graduate of an approved school of nursing with current registration in Massachusetts.
- Minimum of 1 year of clinical nursing experience in an ambulatory clinic/practice setting.
- Experience with telephone triage and telephone-based clinical assessment techniques.
- Experience providing in-person triage and direct patient care for acute and chronic conditions in an ambulatory outpatient setting.
- Experience performing point of care testing and ambulatory clinic procedures.
- Demonstrated evidence of exemplary nursing practice and experience in multiple ambulatory specialties.
- Case Management experience.
- Experience with team-based care.
