Key Responsibilities
  • Perform initial evaluations within the first two weeks of a new resident's stay to assess their capacity to manage Activities of Daily Living and to understand their preferences.
  • Coordinate additional required services such as homemaking, transportation, meal delivery, and personal assistance.
  • Conduct at least annual follow-up assessments.
  • Record and communicate any changes in residents' abilities.
  • Collaborate with family members and residents to address their needs.
  • Engage with discharge planners and rehabilitation services for residents returning from hospitalization.
  • Provide residents with information and referrals to government-supported services, including Nursing Home Services, Medicare/Medicaid Benefits, State Health Insurance Program, Medicaid Spend-Down Program, Food Stamps, QMB, and Circuit Breakers.
  • Work alongside service providers to facilitate social, educational, and health-related activities.
  • Act as an advocate for residents when necessary.
Compliance and Training
  • Complete 36 hours of approved training within the first year of employment.
  • Fulfill 12 hours of approved training annually thereafter.
  • Adhere to safety protocols and procedures.
  • Follow regulations regarding the protection, use, and disclosure of client and organizational information, including HIPAA and PII compliance.
  • Participate in mandatory training sessions.
  • Report any violations of laws, regulations, or policies.
  • Notify authorities of any suspected client abuse or neglect.
  • Ensure timely, accurate, and complete documentation.
  • This role includes responsibilities related to HIPAA compliance.
  • Exposure to bloodborne pathogens is not expected in this position.

Education and Experience
  • Bachelor's degree in social work, gerontology, or a related field.
  • Bilingual proficiency in Russian and English is preferred for applicable positions.
  • A minimum of two years of relevant experience.
Language Proficiency
  • Ability to read, analyze, and interpret business publications, professional journals, technical guidelines, or governmental regulations.
  • Capability to write reports, business correspondence, and procedural manuals.
  • Skill in effectively presenting information and responding to inquiries from groups of managers, clients, customers, and the public.
