Job Summary

This role involves coordinating patient care through our care management program, working closely with healthcare providers and teams to ensure seamless care coordination.

Main Responsibilities
  1. Program Implementation: Develop and implement effective care management programs, identifying areas for improvement and implementing strategies to enhance program outcomes.
  2. Patient Enrollment: Coordinate patient enrollment in care management programs, fostering strong relationships with patients and their families.
  3. Care Coordinator Scheduling: Schedule patients to care coordinators, ensuring an equitable distribution among team members, promoting efficient use of resources.
  4. Collaboration and Communication: Work closely with healthcare providers, including physicians, nurses, and care teams, to ensure seamless care coordination for patients enrolled in the care management program.
  5. Reporting and Analysis: Generate reports and analyze data related to program outcomes, patient satisfaction, and adherence to quality standards, informing strategic decisions.
  6. Compliance and Quality Assurance: Ensure that the care management program complies with all relevant regulations and industry standards, maintaining the highest level of quality and integrity.
Qualifications and Requirements
  • Customer Service and Communication: Provide exceptional customer service and communication skills, responding to patient inquiries and concerns in a timely and professional manner.
  • EMR Systems and Regulatory Knowledge: Familiarity with EMR systems and strong understanding of governmental regulations and Medicare eligibility requirements, ensuring compliance and adherence to industry standards.
  • Organizational and Time Management Skills: Excellent organizational, time management, and problem-solving skills, prioritizing tasks and managing multiple projects simultaneously.
  • Selling and Communication Abilities: Effective selling and communication abilities, promoting the care management program and building strong relationships with patients, families, and healthcare providers.
