Job Summary

We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at Moses/Weitzman Health System. As a Primary Care Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals and families in our community.

Key Responsibilities
  • Provide comprehensive primary nursing care to patients, including health education, disease management, and coordination of care.
  • Collaborate with primary care providers to identify patient needs and develop individualized care plans.
  • Conduct health assessments, develop and implement care plans, and evaluate patient outcomes.
  • Administer medications, including vaccines and immunizations, as ordered by prescribing providers.
  • Perform a wide range of nursing activities, including wound care, catheterization, and other procedures as needed.
  • Accurately document all aspects of patient care in the medical chart.
  • Monitor and ensure follow-up of abnormal test results, including Pap smears, STDs, and other screenings.
  • Oversee the maintenance of supplies and equipment, ordering supplies as necessary.
  • Act as a resource person for medical assistants regarding patient preparation, needs, and procedures.
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of Connecticut.
  • Experience in primary care nursing, preferably in a community health setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
  • Basic computer skills, including proficiency in electronic health records.
