Primary Care Nurse

We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at Moses/Weitzman Health System. As a Primary Care Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals and families in our community.
Key Responsibilities- Provide comprehensive primary nursing care to patients, including health education, disease management, and coordination of care.
- Collaborate with primary care providers to identify patient needs and develop individualized care plans.
- Conduct health assessments, develop and implement care plans, and evaluate patient outcomes.
- Administer medications, including vaccines and immunizations, as ordered by prescribing providers.
- Perform a wide range of nursing activities, including wound care, catheterization, and other procedures as needed.
- Accurately document all aspects of patient care in the medical chart.
- Monitor and ensure follow-up of abnormal test results, including Pap smears, STDs, and other screenings.
- Oversee the maintenance of supplies and equipment, ordering supplies as necessary.
- Act as a resource person for medical assistants regarding patient preparation, needs, and procedures.
- Current licensure as a Registered Nurse (RN) in the state of Connecticut.
- Experience in primary care nursing, preferably in a community health setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on patient-centered care.
- Basic computer skills, including proficiency in electronic health records.
