Job Opportunity:
LPN Care Coordinator
Starting Pay:
Competitive rates based on experience
Differentials Offered:
Shift differentials for evening and night shifts, as well as weekends
Additional Shift Differential:
Extra compensation for shifts worked outside of scheduled hours
Available Shifts:
32-hour night shift
Responsibilities:
The LPN Care Coordinator plays a crucial role in planning, executing, and supervising nursing services for assigned residents. This includes adhering to quality care standards, facility policies, legal requirements, medical practices, and physician directives.
Key Duties:
1. Efficiently plans and delivers professional care while aligning with the needs of residents and the evolving medical landscape.
2. Maintains professionalism in interactions with residents, physicians, families, peers, and support staff.
3. Accurately documents essential information related to resident care.
4. Administers prescribed medications and ensures precise record-keeping in accordance with facility policies.
Requirements:
1. Valid state nursing license in good standing (or in the process of becoming a registered nurse).
2. Comprehensive knowledge of nursing practices, medical procedures, and long-term care regulations.
3. Strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks effectively.
4. Collaborative team player with prior experience in long-term care preferred.
Additional Expectations:
All LPN positions require weekend on-call availability to support both staff and residents. Compensation details available upon inquiry. Per diem staff are expected to fulfill weekend shifts monthly to meet requirements.
