Pursue a challenging and rewarding career as a Hospice Nurse Practitioner with our organization. As a key member of our interdisciplinary team, you will provide expert consultation in palliative care, symptom management, and supportive care to meet the unique needs of our Hospice patients.
In this role, you will have the opportunity to make face-to-face visits within prescribed time limits to qualifying Hospice patients, reporting clinical findings to our Hospice Medical Director. You will also consult with the Hospice Medical Director as needed, inform primary physicians of recommended services, and collaborate with other physicians as necessary.
The ideal candidate will possess a strong background in oncology, hospice, geriatrics, or intensive care nursing, with a minimum of five years of experience. Current certification as a clinical Nurse Specialist or Nurse Practitioner is required, along with current CPR certification. A driver's license and vehicle insurance are also necessary for reliable transportation to patient visits.

Responsibilities
- Consult with the Hospice Medical Director as needed.
- Maintain accurate and relevant clinical notes regarding patient conditions.
- Collaborate with patients, families, attending physicians, and other members of the IDG in providing patient and family care.
