Essential Duties
The successful candidate will be responsible for:
- Providing comprehensive medical care to patients in their homes.
- Conducting thorough assessments of patients' physical and emotional needs.
- Developing and implementing individualized care plans to meet the unique needs of each patient.
- Collaborating with other healthcare professionals to ensure seamless care delivery.
- Providing education and support to patients and their families.
Requirements
To be considered for this role, you will need to have a Masters or Doctoral Degree as a Nurse Practitioner. You must also hold a valid active state nursing license in good standing. A minimum of 3 years' experience in acute care medical/surgical nursing is required, as well as 2 years' experience in managing wound, ostomy, and continence care. Board Certification in any of the following areas is preferred: Adult Nurse Practitioner, Family Nurse Practitioner, Geriatric Nurse Practitioner, or Clinical Nurse Specialist. Basic Life Support certification from the American Heart Association is also required. Wound Care Certification is preferred but not mandatory.
