About the RoleWe are looking for a compassionate and skilled LPN to provide high-quality nursing care to our patients in the comfort of their own homes. As an LPN with Affirmed Home Care, you will have the opportunity to work with a diverse range of patients and develop a deeper understanding of their unique needs and challenges.
Responsibilities- Provide "Best Practice" nursing care to patients in their homes
- Travel to patient locations to deliver care
- Document patient information in a complete and confidential manner
- Evaluate patient wellbeing and report any needs to the Director of Clinical Nursing or emergency services
- Provide personal care and meals to patients
- Perform more advanced nursing tasks, such as chest tubes, IVs, colostomy, and urinary catheter management
- Offer emotional support to clients and their families in times of crisis
- Valid LPN license
- Annual physical within one year
- PPD or Quantiferon within one year or chest x-ray within five years
- COVID vaccine card
