As a Case Manager, you will have a vital role in enhancing the lives of our clients by fostering an environment conducive to long-term recovery. You will collaborate with the Clinical Director and Executive Director to create and sustain a supportive therapeutic environment for our residents.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling, along with 1 year of experience in substance abuse care or clinical experience with individuals facing co-occurring disorders (SUD and mental health).
  • Registered Nurse with 1 year of direct experience in providing services to individuals with substance use disorders or experience with co-occurring diagnoses.
  • CSAC or CSAC-Assistant certification.
  • Valid Driver's License.
  • Willingness to travel up to 25% of the time.
  • Availability to work overtime as required.

Key Responsibilities:

  • Delivering case management and care coordination services for clients enrolled in treatment, ensuring effective resource utilization and compliance with external agency requirements.
  • Facilitating individual and family educational sessions and managing treatment and discharge coordination as necessary.
  • Timely communication of relevant issues or activities to the team and maintaining strong relationships within the treatment environment.
  • Adhering to organizational policies and maintaining confidentiality in accordance with industry standards.
  • Executing additional duties as assigned.
