Job Title: Resource Specialist

Job Summary:

We are seeking a highly motivated and detail-oriented Resource Specialist to join our team. In this role, you will provide resource support to members, connect them to local resources, and work with leadership to identify the best resources for their needs.

Key Responsibilities:
  • Assess member needs and develop care plans/service plans in accordance with their goals and preferences.
  • Connect members to local resources for various services, such as employment, housing, independent living, justice, foster care, and community transitions.
  • Work with providers, community-based organizations, and governmental entities to support members and their families in accessing essential care/services and resources.
  • Develop education and training programs for care management staff and providers to improve resource service identification.
  • Ensure all resource services information is appropriately documented and up-to-date.
Requirements:
  • Bachelor's degree and 2-4 years of related experience.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Knowledge of local resources and services, particularly in the Kansas area.
