Senior Health Advocate

Your primary responsibility will be to develop and implement individualized care plans that address the unique needs of each member. You will work closely with internal teams, providers, and community resources to ensure seamless care coordination and optimal health outcomes. Additionally, you will engage members face-to-face and/or telephonically to complete comprehensive needs assessments and provide education on available resources and services.

To succeed in this role, you will need to possess a Master's degree in social work or a related field, or a bachelor's degree in social work with relevant experience. A current, unrestricted independent licensure as a Registered Nurse in Indiana is also required. Strong communication and interpersonal skills are essential, as well as experience working with local behavioral health providers and community support organizations addressing social determinants of health (SDoH).
Key Responsibilities:
- Partner and collaborate with internal care team, providers, and community resources/partners to implement care plan
- Provide referral and linkage as appropriate and accepted by member (may include internal consult opportunities such as Housing Navigator, Pharmacy Team, Peer Specialist, etc. or community-based provider referrals such as PCP, specialists, medication assisted therapy referrals, etc.)
