Key Responsibilities- Provide clinical oversight and coordination of patient care services
- Evaluate patient referrals for appropriateness and ensure timely admissions
- Develop and implement individualized patient care plans
- Assure the coordination of patient care delivery and continuous assessment of patient needs
- Participate in quality improvement initiatives and maintain compliance with regulatory requirements
- Collaborate with interdisciplinary teams to achieve clinical and operational goals
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
- Two (2) years recent practical experience as an RN
- One (1) year experience in Home Health required
