PruittHealth @Home is seeking a compassionate and experienced Clinical Manager to lead our Home Health team. As a Clinical Manager, you will provide clinical oversight and guidance to our team of skilled clinicians, ensuring the highest quality patient care and outcomes.
Key Responsibilities:
- Coordinate referrals with internal and external sources to ensure seamless patient care delivery.
- Screen patient referrals for appropriateness and evaluate the agency's ability to meet clinical needs.
- Provide oversight on patient and partner assignments, ensuring optimal care delivery.
- Develop, implement, and update individualized patient care plans to meet unique needs.
- Collaborate with the Administrator to ensure effective patient care delivery and agency operations.
- Participate in center/agency surveys and maintain accurate records.
- Utilize reporting capabilities to achieve desired outcomes for patients, billing, and financial success.
- Coach, mentor, and serve as a preceptor to clinical partners to achieve positive recruitment and retention objectives.
Requirements:
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
- Two (2) years recent practical experience as an RN.
- One (1) year experience in Home Health required.
