We are seeking a highly skilled and compassionate Geriatric Care Specialist to join our team at Wellbe Senior Medical Careers. As a key member of our interdisciplinary team, you will play a critical role in delivering high-quality, patient-centered care to our elderly patients.
Key Responsibilities- Provide primary care in the home-based setting for patients, focusing on the care of the frail, poly-chronic, elderly Medicare Advantage patients.
- Partner with patients' primary care physicians to provide concierge-level geriatric medical care and social support in the home.
- Deliver and coordinate care across the entire continuum, from chronic care and urgent care in the home to hospital, skilled nursing facility, assisted living, palliative care, and end-of-life care.
- Ensure compliance with state and federal regulatory agencies and certified agencies for the region.
- Document patient records accurately and timely, supporting our longitudinal model and transitioning patients to our longitudinal care Nurse Practitioners.
- Previous experience in home care, primary care, hospice, palliative care, or geriatrics is strongly preferred.
- Valid driver's license, car insurance, and access to a car are required.
- Proficiency in Microsoft Office, Electronic Health Records, Scheduling Software, and Ring Central applications is essential.
- Experience in diagnosing and treating ongoing, chronic, and urgent medical/psychological conditions of older adults is required.
