Job Title: Advanced Practice Provider

We are seeking a highly skilled Advanced Practice Provider to join our team at Wellbe Senior Medical. As an Advanced Practice Provider, you will play a vital role in delivering high-quality, patient-centered care to our geriatric patients in their homes.

Job Summary:

The Advanced Practice Provider will be responsible for providing primary care to patients in their homes, including conducting initial patient screenings, creating assessment plans, and completing documentation in a timely manner. The ideal candidate will have experience in geriatric care, home-based care, and palliative care, and will be able to work independently with minimal supervision.

Key Responsibilities:
  • Provide primary care to patients in their homes, including conducting initial patient screenings, creating assessment plans, and completing documentation in a timely manner.
  • Collaborate with other clinicians in the care community to manage the health and wellbeing of patients living in their homes.
  • Facilitate staff, patient, and family decision-making by providing educational resources.
  • Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents.
  • Update patient records and check records for accuracy at each patient appointment.
  • Deliver the highest standard of quality care while maintaining all company metrics pertaining to appropriate patient needs.
Requirements:
  • Master's Degree in Nurse Practitioner studies from an accredited, master's-level NP program.
  • Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the posted state and must reside in state.
  • Current board certification, either AANC or AANP.
  • BLS certified and an active NPI# as an NP.
  • CPR certification.
  • Active, unencumbered DEA licensure or eligible to apply for upon hire.
  • Previous experience in home care, primary care, hospice, palliative care, or geriatrics strongly preferred.
Work Environment:

The Advanced Practice Provider will work in a home-based care setting, visiting patients in their homes and providing care in a variety of residential settings. The ideal candidate will be able to work independently with minimal supervision and will have excellent communication and interpersonal skills.
