We are seeking a highly motivated and experienced LPN Patient Educator to join our team at Community Health Centers. As a member of our interdisciplinary team, you will play a critical role in improving health outcomes for patients with chronic diseases such as diabetes, hypertension, and asthma.

About the Role

The Patient Educator works closely with patients and healthcare providers to educate and coordinate services for chronic disease self-management. Your primary responsibilities will include:

  • Acting as an advocate for patients' care needs by identifying opportunities for care interventions and addressing deficits and gaps in care.
  • Coordinating care services for patients with chronic diseases in accordance with established clinic policies and procedures.
  • Implementing targeted strategies to improve health outcomes, functional or quality of life outcomes, such as disease management or pharmacy management.
Qualifications

To be successful in this role, you must possess:

  • A high school diploma or equivalent.
  • Completion of an accredited Licensed Practical Nursing program.
  • A minimum of 1 year experience as a Licensed LPN in Healthcare or Managed Care.

  • $65,000 - $75,000 per year (estimated salary based on location).
  • No weekends for the majority of our centers, 10 Paid Holidays, and early Fridays.

Requirements

Please note that you must be able to work flexible hours and other CHC locations as needed. Bilingual skills in English and Spanish are also helpful but not required.
