Care Team Nurse Specialist

About the Role:
">- As a Care Team Nurse at Community Health Centers, you will play a vital role in delivering high-quality patient care and supporting our team of healthcare professionals.
">
Key Responsibilities:
">- Coordinate patient care activities and ensure seamless communication between care team members and providers.
- Assist with pre-visit planning, huddles, and one-on-one communication to ensure effective patient care coordination.
- Participate in team meetings to address patient care and operational requirements.
">
Routine Clinical Duties:
">- Manage patient schedules, including work-ins, walk-ins, and open-access appointments.
- Collect patient history and gather relevant information for visits.
- Document all communication with patients and clinicians.
- Administer immunizations and medications as ordered, following safe medication administration practices.

Additional Responsibilities:
">- Monitor quality control logs within your department.
- Track and report on immunizations and injections.
- Maintain supply inventories and medical stock in exam rooms and nursing stations.
">
Requirements:
">- Current/valid Florida LPN license.
- BLS certification or must be obtained within 30 days of employment.
- Minimum 2 years' experience as a licensed LPN in healthcare, managed care, physician practice, or clinic setting.
