Job Summary

The Patient Educator is a vital member of our interdisciplinary team, dedicated to improving health outcomes for patients with chronic diseases. This role requires effective coordination of care services, patient education, and preventive care initiatives.

Key Responsibilities:
  • Act as an advocate for patients' care needs, identifying opportunities for interventions and addressing deficits in care.
  • Coordinate care services for patients with chronic diseases such as diabetes, hypertension, and asthma, adhering to established clinic policies and procedures.
  • Implement targeted strategies to improve health outcomes, functional status, or quality of life, including disease management and patient teaching plans.
  • Monitor hospitalizations, emergency visits, institutional facility admissions, and re-admissions to identify issues and implement follow-up actions for improved outcomes.

Requirements:
  • A minimum of two (2) years' experience as a Licensed LPN or RN in healthcare or managed care.
  • Practical experience in a patient educator role or evidence of training in patient education.
  • Proven ability to function effectively as a member of an interdisciplinary team and demonstrate excellent verbal and written communication skills, case management services.
