Senior Community Care RN Opportunity

We are seeking a skilled and compassionate Registered Nurse to join our Senior Community Care team. As a key member of our interdisciplinary care team, you will play a vital role in coordinating the delivery of high-quality care to our most vulnerable patients.

Key Responsibilities:
  • Ensure accurate and complete documentation of patient care
  • Collaborate with the care team to develop and implement individualized care plans
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
  • Assist with benefit determination and coordination of care services
  • Participate in facility-based meetings and contribute to quality improvement initiatives
Requirements:
  • Associate degree or higher in Nursing (RN)
  • Active and unrestricted license in the state of practice
  • Certified in Basic Life Support
  • Ability to complete physical requirements of the role

We offer a competitive hourly rate of $28.03 to $54.95 per hour, depending on experience and qualifications.
