Nurse Care Coordinator

The Community Health Specialist is responsible for planning, organizing, and directing home care services. This role requires a Registered Nurse with experience in community health education and a focus on patient care.
Key Responsibilities
Plan and direct services to meet the needs of individuals and families within their homes and communities.
Build from the resources of the community to provide comprehensive care.
Collaborate with healthcare teams to ensure seamless care transitions.
Provide education and support to patients and families on health management and disease prevention.
Maintain accurate records and documentation of patient care.
Requirements
Registered Nurse (RN) with current and unencumbered license to practice professional nursing in the state of employment.
Bachelor's degree in Nursing from an accredited program, preferred.
One (1) to two (2) years of recent acute care experience in an institutional setting.
One (1) year of home health care experience, preferred.
Management experience not required, but direct supervisory experience of Licensed Vocational Nurses and Nurse Aides/Assistants, preferred.
Possess and maintain valid CPR certification.
Working knowledge of OASIS documentation requirements, preferred.
Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
Access to reliable transportation.
