LPN - Licensed Practical Nurse

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents of all ages in our long-term care facility. As a member of our interdisciplinary care team, you will work closely with healthcare professionals to deliver compassionate and individualized care.
Responsibilities- Provide professional nursing care to residents, including administering medications, treatments, and performing testing to evaluate resident status.
- Contribute to the assessment of residents and develop care plans that meet their unique needs.
- Collaborate with the care team to ensure the health, comfort, and safety of residents.
- Document resident interactions and outcomes related to care, medication, behavior, activity, and dietary needs.
- Communicate effectively with residents, family members, and healthcare providers to ensure seamless care coordination.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing board and/or possess multi-state licensure privileges.
- Obtains and maintains required department-specific competencies and certifications.
