Job Title: Housing Navigator/Case Manager

At Titanium Healthcare, we are seeking a highly motivated and compassionate Housing Navigator/Case Manager to join our team. As a key member of our organization, you will play a vital role in providing support and assistance to individuals and families experiencing homelessness or at-risk of losing their housing.

Job Summary

The Housing Navigator/Case Manager will be responsible for researching and establishing a list of available housing opportunities in the region, developing relationships with local community resources, and creating individualized housing plans for our members. You will work closely with our team to ensure effective service delivery and provide support to members in understanding tenant rights and responsibilities.

Key Responsibilities
  • Research and establish a list of available housing opportunities in the region
  • Develop relationships with local community resources, including transitional housing, shelters, and permanent housing
  • Create individualized housing plans for members, ensuring effective service delivery and consistency with the member's overall care plan
  • Assist members with their housing search, including applying for apartments, negotiating with potential landlords, and filling out rental applications
  • Provide support to members in understanding tenant rights and responsibilities
  • Conduct follow-up meetings with members after housing placement to address any issues or concerns
  • Maintain accurate documentation of service objectives, outcomes, and member-related information in our data systems
  • Participate in collaborative team meetings to provide input, support, and brainstorm solutions to common areas of concern and develop best practices
Requirements
  • Fluent in English (written and verbal)
  • Competent with computers, email, virtual platforms, Excel, and other Microsoft Office-based programs
  • Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear and concise manner
  • Ability to identify problems and use logic and related information to develop and implement solutions
  • Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures
Preferred Qualifications
  • Minimum 1-2 years of experience in healthcare, social work, public health, or a related field
  • Minimum 1-2 years of relevant experience in social work, case management, working with unhoused and unsheltered individuals/families, and veterans' programs
  • Experience in a social service setting with working knowledge of case management systems and planning techniques
  • Current and valid Driver's License
  • Proof of auto insurance
