About the Job

Sun River Health is a Federally Qualified Health Center (FQHC) providing comprehensive primary, dental, pediatric, OB-GYN, and behavioral health care to over 245,000 patients annually. We are seeking a dedicated Patient Navigator to join our team at our Southampton site.

Key Responsibilities
  • Consult with medical staff to identify patients in need of preventive and primary health care services and link them to the services they need.
  • Facilitate patient appointments within different departments of SRH and outside agencies.
  • Track progress in eCW and alert providers.
Requirements
  • High School Diploma/GED required.
  • 2-4 years of related work experience.
Compensation

$19.00 - $21.00 per hour.
