About the Role: As a Patient Navigator, you will be responsible for identifying individuals and families who require preventive and primary health care services and connecting them with the resources they need.

Key Responsibilities:

  • Collaborate with medical staff to provide translation, transportation assistance, and application support for patients.
  • Ensure seamless communication between SRH departments and outside agencies, following established procedures for referrals.
  • Empower patients to overcome obstacles hindering their access to necessary referrals.
  • Monitor patient progress and update the clinical team on open referrals.
  • Provide accurate information to patients regarding provider instructions and referral processes.
  • Manage clinical data transmission to specialists and ensure proper consent for information release.

Essential Skills: A High School Diploma or GED, 2-4 years of relevant work experience, strong interpersonal skills, and proficiency in electronic health records systems are required.

Compensation and Benefits: This full-time position comes with a salary range of $19.00 - $21.00 per hour and a comprehensive benefits package aligning with industry standards.
