As a Care Manager RN, you will oversee patient care, ensuring quality and integrating services. You will use standards to manage benefits and justify service necessity. This role combines healthcare practitioner, case manager, and educator duties to achieve top clinical outcomes.
ResponsibilitiesKey responsibilities include:
- Assessing patient care plans and documenting, executing, and updating collaboratively with the healthcare team
- Constructing patient care management strategies, treatments, and self-management priorities
- Evaluating healthcare, educational, and psychosocial requirements of patients and their families
- Helping patients manage health autonomously through education, in-person visits, and remote engagement
- Exploring, assessing, and recommending resources for medical and non-medical needs of patients and families
- Uniting with all programs for service coordination
- Identifying and addressing problems in care coordination, exploring potential solutions to ascertain the most suitable care setting
- Serving as a liaison to hospitals, general care providers, post-recovery partners, and specialists
To be successful in this role, you should possess:
- Proficiency in conducting patient assessments and managing their care
- Exceptional verbal and written communication skills
- The ability to diligently monitor, evaluate, and document patient progress with respect to personalized care plans
- The capability to facilitate a patient's access to appropriate community resources
- Strong partnership skills, with the ability to work well with internal medical teams and external partners and organizations
- A flexible capability to evaluate, adjust, and respond positively to a shifting and crisis-filled environment
- Respect for all aspects of patient confidentiality, in compliance with all relevant policies and regulations

QualificationsTo be considered for this role, you should have:
- A minimum of an Associate's degree
- A minimum of two years relevant work experience
