Job Summary:

Baylor Scott & White Health is seeking a skilled Care Manager RN to oversee patient care, ensuring quality and integrating services. This role combines healthcare practitioner, case manager, and educator duties to achieve top clinical outcomes.

Key Responsibilities:

  • Assess patient care plans, document, execute, and update collaboratively with the healthcare team to deliver evidence-based care.
  • Construct patient care management strategies, treatments, and self-management priorities.
  • Evaluate healthcare, educational, and psychosocial requirements of patients and their families.
  • Help patients manage health autonomously through education, in-person visits, and remote engagement.
  • Explore, assess, recommend resources for medical and non-medical needs of patients and families.
  • Identify and address problems in care coordination, exploring potential solutions to ascertain the most suitable care setting.
  • Serve as a liaison to hospitals, general care providers, post-recovery partners, and specialists.

Requirements:

  • Proficiency in conducting patient assessments and managing care is critical for this role.
  • Ability to diligently monitor, evaluate, and document patient progress with respect to personalized care plans.
  • Capability to facilitate a patient's access to appropriate community resources.
  • Flexibility to evaluate, adjust, and respond positively to a shifting and crisis-filled environment.
  • Respect for all aspects of patient confidentiality, in compliance with all relevant policies and regulations.
