Key Responsibilities- Provide patient care and support in the home environment, including administering medications, monitoring vital signs, and performing various nursing procedures.
- Collaborate with healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical and mental conditions, documenting any changes or concerns.
- Administer medications as prescribed by physicians and document patient files accordingly.
- Participate in multi-disciplinary patient case conferences and staff meetings to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care outcomes.
- Graduate from an accredited school of practical nursing.
- Licensed as a vocational/practical nurse in the state of practice.
- At least one year of clinical practical nursing experience, preferably in intensive care, rehabilitation, or medical surgical nursing.
- Home health experience preferred.
- Ability to make home visits and provide nursing care in the home environment.
- Ability to complete necessary documentation accurately, legibly, and timely.
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
