Job Summary

Telecare is seeking a skilled Case Manager I to join our team at Canyon Vista, a transitional and permanent supportive housing program serving adults with serious mental illness in San Mateo County.

Key Responsibilities
  • Provide support and case management services to members served with mental illness and other co-occurring disorders
  • Assist members in achieving their recovery goals and living independently in their chosen community
  • Collaborate with multidisciplinary teams to develop and implement individualized service plans
  • Document and communicate interactions with members to other service team members as appropriate
  • Assist members in accessing community resources and benefits
Requirements
  • High School Graduation or equivalent
  • Demonstrated sensitivity to multicultural populations and issues
  • At least years of age
  • CPR, Crisis Prevention Institute (CPI), and First Aid certification
  • Valid driver's license and ability to drive
Preferred Qualifications
  • Experience in mental health and/or dual diagnosis settings
Working Conditions

The physical demands of this position include standing, twisting, squatting, kneeling, pushing, pulling, and lifting items weighing pounds or less. The position requires frequent driving and hazardous exposure to blood-borne pathogens, hospital waste, chemicals, and infections.
