Job Summary:Under the supervision of our experienced team, you will serve as a primary support and case manager for our members, assisting them to live as independently as possible in their chosen community. Your expertise and empathy will help our members achieve their goals and improve their overall well-being.
Responsibilities:- Collaborate with members to create personalized Recovery Plans
- Provide case management services, including housing stabilization and rehabilitative support
- Facilitate groups and provide individual support to meet the unique needs of our members
- Assist with crisis situations and provide interventions as needed
- Collect and document required data, ensuring accurate and timely reporting
- Coordinate transitions to lower levels of care and connect members with community resources
- Assist with daily living tasks and provide support with appointments and benefits
- Ensure confidentiality and adhere to HIPAA policies
- High School diploma or equivalent and 2 years of direct services experience in mental health or community services
- Sensitivity to diverse populations and issues
- CPR, Crisis Prevention Institute (CPI), and First Aid certification
- Valid driver's license and ability to drive safely
- 4 years of experience in a mental health setting
- Experience in providing intensive case management and community supports
Canyon Vista is a 57-unit transitional and permanent supportive housing program serving adults in San Mateo County. Our program provides 24/7 support and services to help our members achieve their goals and improve their overall well-being.
