About This Role

You will build strong bonds and lasting relationships with patients and their families, working collaboratively with the interdisciplinary team to enhance patient care outcomes and facility operations. The role requires decision-making, teaching, leadership functions, and quality improvement activities.

Key Responsibilities
  • Coordinate patient care, including assessment, planning, intervention, and evaluation, for an assigned group of hemodialysis patients.
  • Analyze patient data and document medical records.
  • Adjust or modify treatment plans as needed and notify supervisors.
  • Provide initial and ongoing education to patients and families.
  • Administer medications as prescribed or in accordance with approved algorithms.
  • Ensure correct laboratory collection, processing, and shipping procedures.
Requirements

To be successful in this role, you should possess:

  • A graduate degree from an accredited School of Nursing.
  • Current state licensure and CPR BLS certification.
  • Experience as a Registered Nurse (preferred) and knowledge of hemodialysis procedures.
