Position Overview:


The Clinical Pharmacist will provide integrated, on-site support to patients and healthcare providers, employing a hands-on approach to ensure comprehensive patient care through effective medication management.


Key Responsibilities:


The Clinical Pharmacist will play a vital role in advancing pharmacotherapy quality by ensuring the delivery of safe, effective, and economical drug therapy for all patients in the oncology setting.

This role involves collaboration with a multidisciplinary team in an outpatient oncology environment, emphasizing a team-based care model.

The pharmacist will engage in direct patient interactions, offering disease management through face-to-face consultations and telephonic support.
Additionally, the pharmacist will review prior authorization requests for medications under the pharmacy benefit.

Regular communication with healthcare professionals and patients regarding medical care and quality of life is essential, with a focus on monitoring and documenting pharmacotherapy outcomes.

The Clinical Pharmacist will consult with patients to identify, address, and prevent medication-related issues.
They will evaluate each patient's medication regimen for appropriateness, effectiveness, safety, and adherence.
Responsibilities include medication therapy management, advising clinicians on evidence-based practices, and documenting interventions.
Reviewing clinical chart information to assess treatment appropriateness is also required.
Monitoring prior authorization requests and developing clinical criteria and medical policies for medication review is essential.

Compliance with regulatory standards from the Center of Medicare and Medicaid Services (CMS) and the New York State Department of Health (NYSDOH) is mandatory.

Additionally, adherence to guidelines from the Food and Drug Administration (FDA) and relevant clinical literature is crucial.

The pharmacist will manage and update relevant data using database applications, applying insights to enhance operational efficiency, productivity, and service quality.
Identifying trends, resolving issues, recommending improvements, and implementing changes are key aspects of this role.
Active participation in process improvement initiatives is expected.
Confidentiality and discretion are paramount in protecting sensitive information.
Continuing education and professional development are encouraged to maintain current knowledge and skills.
Other responsibilities may be assigned as needed.

Qualifications:

Licensure/Certification:
Candidates must possess a Doctor of Pharmacy (PharmD) degree and be a licensed pharmacist in their state of employment.

Experience:
A minimum of 1 year of experience as a clinical pharmacist is required, along with completion of a PGY1 residency. A PGY2 residency specializing in oncology is also required.

Skills:

Strong analytical abilities, sound judgment, and excellent verbal communication skills are essential. A deep understanding of pharmacological advancements and FDA regulations is necessary.

Ability to manage processes effectively, demonstrating strong technical and clinical skills while maintaining a focus on legal compliance and quality standards is critical.

Proficiency in computer applications is required, with knowledge of pharmacy software systems being advantageous. A solid understanding of specialty pharmaceuticals and billing practices is also necessary.

Candidates should be organized, dependable, and detail-oriented, with a strong focus on quality and empathy. Excellent listening and communication skills are essential.
