Job Summary:

Sunrise Senior Living is seeking a highly skilled and compassionate LPN Medication Care Manager to join our team. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care and services for our residents. Your primary responsibility will be to administer medications and treatments in a safe and organized manner, while also providing resident care and promoting dignity and physical safety.

Responsibilities:

  • Administer medications and treatments to residents in a safe and organized manner
  • Document medication administration and ensure accurate records
  • Conduct Service and Health Updates as directed by the Resident Care Director
  • Participate in the development of Individualized Service Plans (ISPs) and monthly updates
  • Transcribe orders and maintain accurate records
  • Partner with the community team to ensure compliance with OSHA and Occupational Health and Safety Regulation requirements
  • Report all accidents, incidents, and unsafe conditions immediately
  • Ensure the proper disposal of bio-hazardous materials and maintain a clean and safe environment
  • Assist in checking medication and restocking medication carts
  • Count narcotics with another Medication Care Manager or Lead Care Manager each shift
  • Maintain and clean the Medication Room, medication carts, and treatment carts
  • Follow re-fill process for medications
  • Help residents maintain independence and promote dignity and physical safety
  • Strive to understand and respond to each resident's unique communication patterns, history, and basic human needs
  • Audit medication carts and maintain accurate records

Qualifications:

  • LPN, LVN, or state/province specific licensed nurse credential
  • Maintain certifications and ongoing training and re-education as required by Sunrise and state/provincial regulations
  • CPR and First Aid certification
  • Ability to make choices and decisions and act in the resident's best interest
  • Ability to react and remain calm in difficult situations
  • Ability to handle multiple priorities
  • Possess written and verbal skills for effective communication and level of understanding
  • Demonstrate good judgment, problem-solving, and decision-making skills
