Medical Director of Senior Care Services

At Abode Care Partners, we are seeking a compassionate and highly skilled Nurse Practitioner to join our team. As a Medical Director of Senior Care Services, you will be responsible for providing evidence-based medical care to seniors and patients with complex needs.
Responsibilities:- Performs comprehensive assessments and develops individualized care plans to meet the unique needs of each patient
- Provides direct patient care, including examination, diagnosis, and treatment of acute and chronic conditions
- Collaborates with interdisciplinary teams to ensure seamless transitions of care and coordination of services
- Orders and interprets laboratory and diagnostic testing as needed, and ensures timely follow-up on results
- Participates in quality improvement initiatives and contributes to the development of policies and procedures to enhance patient care and outcomes
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Strong interpersonal communication skills – verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow

We offer a competitive salary range of $100,000 - $120,000 per year, plus benefits including:
