About the Role

We are seeking a highly skilled Nurse Practitioner to join our team at Abode Care Partners, where you will provide exceptional medical care to seniors and individuals with complex needs.

Key Responsibilities
  • Deliver evidence-based medical care by evaluating and treating chronic and urgent health issues.
  • Collaborate with healthcare professionals to develop comprehensive plans of care and ensure accurate patient records.
  • Examine and treat patients, including ordering laboratory and diagnostic testing as needed.
  • Provide referrals to other healthcare practitioners when necessary.
Requirements
  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
