Medical Care Specialist for Seniors

We are seeking a highly skilled Nurse Practitioner to join our team at Abode Care Partners, where you will provide exceptional medical care to seniors and individuals with complex needs.
Key Responsibilities- Deliver evidence-based medical care by evaluating and treating chronic and urgent health issues.
- Collaborate with healthcare professionals to develop comprehensive plans of care and ensure accurate patient records.
- Examine and treat patients, including ordering laboratory and diagnostic testing as needed.
- Provide referrals to other healthcare practitioners when necessary.
- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
