Job Description

We are seeking a skilled Registered Nurse to join our Hospice Program at Constellation Health Services. As a compassionate caregiver, you will provide high-quality patient care in the comfort of their own homes, assisted living facilities, and skilled nursing facilities.

Responsibilities:

  • Provide skilled nursing care, including medication administration and treatments as prescribed by the physician's plan of care.
  • Conduct comprehensive assessments and coordinate care plans with all disciplines associated with the patient's needs.
  • Observe, evaluate, and report on patient physical and emotional status to the nursing supervisor and attending physician.
  • Participate in weekly Interdisciplinary Team Meetings to provide updates on patient status.
  • Maintain and process all documentation in compliance with agency standards.

Requirements:

  • Graduate of an accredited RN program.
  • Registered Nurse Licensed in the state of Maine.
  • At least 1-2 years of RN experience.
  • Previous Hospice care experience strongly desired.
