Hospice RN Case Manager

The Registered Nurse Case Manager delivers care to patients utilizing the nursing process of assessment, interventions, implementation, and evaluation. He/she contributes to an interdisciplinary group to enhance quality of hospice care and case manages all assigned patient clinical records.
Key Responsibilities:
- Comply with all Federal, State, Local laws and abide by Medicare Conditions of Participation guidelines as set forth by Center for Medicare & Medicaid Services (CMS).
- Uphold the mission, vision, and values of SouthEast Hospice.
- Develop & maintain community relationships and always present a positive image.
- Perform as a member of the SouthEast Hospice team as a whole and participate in the total quality management philosophy of the agency.
- Assume primary responsibility for case management of all assigned patients by assessing and evaluating the patient's condition, controlling symptoms and observing any issues regarding the patient and family. Reports any issues or changes to hospice staff and physician as needed.
- Provide direct care to patients as prescribed in the interdisciplinary plan of care in order to maintain the highest level of comfort and quality of life.
- Coordinate community resources and other agency disciplines participating in patient care.
- Knowledge of physical assessment, completion of all pertinent documentation and communication to SouthEast team.
- Complete comprehensive patient assessments to identify needs and provides appropriate care as supported by documentation and patient care plan.
- Teach primary caregiver(s) how to safely operate equipment, such as suction machines, Hoyer lifts, nebulizers, oxygen tanks/concentrators, bedside commodes, and other physical equipment as needed.
- Responsible for implementing patient care to support terminally ill patients and families in personal homes or nursing facilities.
- Knowledge of referral and admission processes, to include evaluation and educational visits that may require explaining the hospice/Medicare benefit.
- Assess home care needs, being aware of physical, emotional, and spiritual aspects and gather data on social, economic, and cultural factors which may influence health, well-being, and quality of life.
- Provide patient and family teaching as needed.
- Provide support to families at time of death.
- Assist patients, family members, or other clients with concern and empathy.
- Respect confidentiality and privacy.
- Communicate in a courteous and respectful manner.
- Attend and participate in interdisciplinary group meetings.

Requirements:
- Graduate of an accredited school of nursing.
- Valid license as a Registered Nurse in the state of Georgia (or reciprocal state).
- Hospice and/or home health experience highly desirable; Minimum one-year recent professional nursing experience.
- Current Basic Life Support Certification.
- Strong leadership skills and work well in a team environment.
- Ability to be sensitive to the needs of terminally ill patients and families and one's own feelings about death/dying.
- Ability to apply knowledge of the special needs of hospice patients & families, knowledge of community resources, and knowledge of proper documentation in the electronic medical record.
- Ability to operate computer, facsimile equipment, Xerox machine, iPad, and cell phone.
- Valid driver's license, a reliable vehicle for work-related purposes, and current vehicle insurance.
- Satisfactory background investigation, medical clearance (to include substance abuse testing), and PPD test is required prior to the start of your employment.

Work Conditions and Physical Capabilities:
- Ability to lift and/or turn patients using appropriate techniques.
- Ability to walk, sit, stand, or climb stairs -- may be for an extended period of time.
- May be exposed to various infections, viruses, and diseases from patients & specimens in working environment.
- May be required to drive for long distances.
- Willingness to visit patient & families in facilities (such as hospitals, long-term care centers, assisted living facilities, personal care homes) and personal homes, being mindful that environmental conditions vary.
