Job Summary

Strong Force Staffing is seeking a dedicated RN Case Manager for a respected hospice care agency in Phoenix. This critical field role involves direct patient care and comprehensive case management, aiming to enhance end-of-life care and improve patient outcomes.

Responsibilities

  • Assess patient conditions and develop individualized care plans focusing on comfort and quality of life.
  • Maintain detailed medical records and communicate patient status to relevant parties, ensuring seamless care coordination.
  • Serve as the primary liaison among patients, their families, and healthcare providers, offering compassionate support and education.
  • Educate patients and families on health management, palliative care options, and preventive measures to manage symptoms effectively.
  • Coordinate with healthcare professionals to ensure continuous, patient-centered care and efficient case management.
  • Conduct regular home visits to monitor patient progress and adjust care plans as necessary.

Requirements

  • Active RN license in Arizona with no restrictions.
  • At least 1 year of in-home hospice experience is required.
  • Proficient in case management software, exceptional organizational and communication skills.
  • Empathetic, detail-oriented, and able to operate autonomously in field settings.
