Job Summary

We are seeking a highly motivated and compassionate Children's Behavioral Health Case Manager to join our team at Intercommunity Action I. As a key member of our youth services team, you will provide critical support to children and adolescents with behavioral health disorders, connecting them with essential resources and services in their community.

Key Responsibilities:

  1. Provide Blended Case Management (BCM) services to a caseload of up to 25 youth and their families, meeting performance and productivity standards.
  2. Engage with, empower, and advocate for assigned youth and families through proactive outreach and home, school, and community visits.
  3. Conduct ongoing assessments of youth and family strengths and needs, developing comprehensive service coordination plans in collaboration with treatment teams.
  4. Link youth and families to identified resources with their consent, facilitating referrals for expanded mental health programming and coordinating care between service providers and systems.
  5. Serve as a clinical liaison, coordinating care and ensuring continuity of services for youth transitioning between levels of care.
  6. Provide 24-hour crisis intervention, coordinating care with mobile crisis teams and psychiatric hospitals as needed.
  7. Maintain accurate records and documentation in compliance with agency, county, state, and managed care regulations.
  8. Collaborate with service providers and participate in professional development opportunities to enhance skills and knowledge.

Requirements:

  1. Meet one of the following criteria: a bachelor's degree in a relevant field, a registered nurse, or a high school diploma with 12 semester credit hours in social science and 2 years of human services experience.
  2. Hold valid Childline, PA Criminal History, and FBI Clearances prior to commencing work.
  3. Have reliable transportation and a valid driver's license from the state of residence with at least one year of driving experience.
  4. Be available to work some evening hours.
